ABSTRACT
Dr. Miller is an internationally recognized expert
on artificial intelligence in medicine, computer-assisted
diagnosis and clinical information systems.
Over the past decade, Vanderbilt University Medical
Center has developed and implemented a new approach
to care provider order entry. Key components have
included: a "clinical informatics" team
approach in which clinicians build and evolve the
system for clinicians; creating a decision-making
environment for the clinician where the right information
is "one click away" and the system writes
orders for the end-user as a result of higher-level
interactions (rather than the clinicians directly
typing what becomes verbatim legible orders); continuous
engagement of end-users in ongoing rapid prototyping
and evolution efforts; and, institutional involvement
in knowledge acquisition and maintenance, as well
as fostering "best practices" through
system enhancements. Preliminary evaluations have
demonstrated beneficial impact on quality of care,
educational resource utilization, and institutional
resource utilization.
