Abstract:
Many patients would like to take a more active role in their own health care. Yet, we cannot expect patients to become fully engaged in the management of their health conditions without providing adequate access to their information and appropriate clinical decision support tools. Dr. Tang will review shared information needs of physicians and patients and how electronic health records and integrated personal health records can enhance physician-patient partnerships in care. He will illustrate the case with electronic tools in use at the Palo Alto Medical Foundation.
